We have the privilege of working with any high-quality landlords across the UK to provide fit-for-purpose, personalised housing for vulnerable individuals in the community.
Your Plan
Together Noble Tree Care and you will agree a Person-Centred Support Plan that keeps at the centre your choices to live safely, look after your wellbeing and support you to actively take part in your community.
Together, we will review your plan to recognise your achievements and to make new goals as the first are achieved.
Our support staff will use your plan to support you and to get to know your needs and wishes ensuring that you always receive the best personalised care.
Your Care
Noble Tree Care is developing a highly skilled team of dedicated and friendly staff who are passionate about the work that they do.
Support workers will support you on a day-to-day basis to meet your goals and enable you to do the things you want to. One of your support workers will become your key worker who will work with you on a one-to-one basis to support you with any medical needs and independence goal setting so you have someone you can always discuss concerns and achievements with.
Your support team will be trained to meet individual needs through bespoke training, internal opportunities for development, and supported to achieve NVQs.
Your Home
Noble Tree Care is a provider of Individualised Supported Living services open to vulnerable adults and young vulnerable adults transitioning from children’s services to living within their communities from the age of 18.
We will guide and support you to develop and maximise independence promoting self-resilience.
If you choose our supported living services, you will have a tenancy agreement directly with the housing provider and support from us to achieve the plans you have made.
